Welcome to the MyJCV Check mobile app! We refer to this mobile app throughout this Privacy Notice as “MyJCV Check App", or the “App”. This App was developed and designed by RxPx with input and advice from Sandoz Pty Ltd (“Sandoz” or “Sponsor”), who sponsors this App. An important feature of this App is the way in which your privacy is maintained. RxPx makes the MyJCV Check App and decides what personal information to collect, use, and share, how the App will work, and how to support members. Sandoz is the Sponsor of the App and provides input and advice to RxPx on the MyJCV Check App design. RxPx does not share personal information from the MyJCV Check App with third parties, including Sandoz.

RxPx knows who you are from the personal information you enter when you sign up for MyJCV patient support program and use the MyJCV Check App.
Required:
Account information – email address, password
Basic information – date of birth, location (country, state), gender
Healthcare provider’s contact information which may infer medical diagnoses
The required basic and personal health information helps RxPx to provide services, including reminders, and relevant content.

RxPx obtains your consent when you sign up to use your personal information for the following purposes when you use the MyJCV Check App:
create the MyJCV Check App account,
facilitate laboratory testing reminders,
deliver curated content in the App,
contact you with updates and information about the MyJCV Check App,
understand how you use the MyJCV Check App so we can improve it,
provide you with responses including technical support for MyJCV Check App when you need it, and
If you choose to use certain features, we will use personal information to help you to contact your health team outside MyJCV Check App.
RxPx uses information provided by you, including personal information, and information about what you do in the MyJCV Check App to create data that does not identify individuals or link individuals to personal information. RxPx uses this anonymous data to understand the needs of the MyJCV Check App community. RxPx shares this anonymous information with Sandoz.
RxPx does not sell, rent, or disclose personal information collected, used, or created in the MyJCV Check App.
RxPx may make personal information known to third parties if:
The law says RxPx must reveal it.
RxPx believes it is necessary to protect your safety, the safety of other members, or the public.
RxPx must reveal personal information to protect and defend our rights or property.

Select RxPx employees will be able to see personal information in the MyJCV Check App. They will see it only if they need to support you or if their work on the MyJCV Check App requires them to see it.
RxPx uses third parties to help deliver the MyJCV Check App to you. These third parties may be able to see personal information. They are only allowed to use personal information to deliver the MyJCV Check App.
RxPx uses these categories of companies in these locations to deliver the MyJCV Check App:
Data hosting, backups and file storage services – United States
Notification and messaging services – United States
Analytics and app usage and performance tracking services – United States
Customer support and ticketing systems – United States
Other users of the MyJCV Check App cannot see the personal information you provide.
The Sponsor cannot see the personal information you provide in the MyJCV Check App.

RxPx keeps your personal information while you are using the MyJCV Check App. If you don’t use the MyJCV Check App for one (1) year, RxPx will remove the account and any associated personal information from the MyJCV Check App active database.
You can also go into settings to cancel your account and ask RxPx to remove personal information from your MyJCV Check App account. RxPx removes personal information by erasing it from the active database. It will take 60 days for RxPx to do this.
RxPx regularly makes copies of the MyJCV Check App for record keeping. RxPx keeps these copies in a safe place for six (6) years. It will take six years from the time personal information is removed from the MyJCV Check App for RxPx to remove personal information from these backup copies.
If you remove personal information from your account or ask RxPx to remove it, you may no longer be able to use the MyJCV Check App. This means you will no longer have access to the features of the app such as laboratory testing reminders.

RxPx is subject to Australian privacy law and other applicable privacy and data protection laws.
We use technical and organizational ways to protect personal information.
RxPx:
Puts personal information into code (“encrypts it”) when moving and storing it.
Removes personal information if there is no need to have personal information.
Makes sure that only employees who need to see personal information to do their jobs can see personal information.
Gets companies helping to deliver the MyJCV Check App to commit to protect personal information. These companies may use personal information only under RxPx’s instructions to deliver the MyJCV Check App.

You have the right to access personal information you have provided in the MyJCV Check App and to withdraw your consent. You can see and edit personal information by opening the settings tab in the App.
You have the right to ask RxPx to correct personal information.
You can cancel your account and ask RxPx to remove personal information from the MyJCV Check App. You can do this by canceling your account on the settings tab in the App.
You can ask RxPx for a copy of the personal information you have provided to us, which we will prepare in a way that is usable if you want to move personal information to other services.
You may have other rights, depending on where you are located.

Yes, RxPx uses automated methods (a set of rules) to deliver curated content that matches the personal information provided when you set up your MyJCV Check App account.

The MyJCV Check App is not designed for use by children without parental or guardian support.

RxPx uses service providers in the United States to process and store personal information.
The email address you use for login will be stored locally on your phone. It is not collected into RxPx databases unless you sign up separately for our marketing communications.

You can tell RxPx’s Privacy Officer at privacy@rxmxcorp.com if you are worried about the MyJCV Check App and personal information.
If telling RxPx’s Privacy Officer doesn’t help, you can contact the Office of the Australian Privacy Commissioner.
We may update this Privacy Notice if we change the way we handle personal information. We will post changes to this page with the date of the change. If we make meaningful changes to the handling of personal information, we will notify you in the App and/or via email.
